For any renewal, it is advised to put down your file 6 months before the maturity date of your rights.


First request and renewal of right s)

To every new file (for a minor or an adult), that your situation is known or no, your file necessarily has to contain the following items (without these your file will have returned to you):

  1. Form one-of-a-kind of request s) Cerfa n°13788*01
    Filled, dated and branded with.

  2. Medical certificate Cerfa n°13878*01
    Filled, dated less than 6 months and branded with by your doctor (compulsory signature and stamp).

  3. A (legible) copy of a documentary evidence of identity:
    French national ID card, residence permit or receipt, family book or birth certificate for the children as well as the ID card of the parents or the legal representative, etc.

  4. A copy of a proof of address:
    E.g.: charge electricity / gas / phone / rental agreement, etc.

    - In case of accommodation at a private individual: enclose a certificate of accommodation and a proof of address in the name of him accommodating.
    Every person hosted in a medical and social establishment has to prove a place of residence of help in the department.

  5. In case of legal protection (supervision / guardianship):
    The copy of the judgment as well as the documentary evidence of identity of the legal representative.

To complete the medical evaluation

Complete your file according to your request:

The processing time of a request depend partially on supplied elements. Your file is complete if it contains items useful for the evaluation of your needs and the instruction of your rights, it is thus desirable that you join these in support of your request. The multidisciplinary team of the MDPH appreciates the comprehensiveness of your file and can accordingly ask you for other items that those listed opposite:

  • Educational allowance of Kid Handicapped person (AEEH) and complement:
    - le Balance sheet of autonomy Filled, dated and branded with by your doctor,
    - The documentary evidences of your expenses bound to the handicap,
    - The documentary evidences of the possible employment of a third person,
    - The documentary evidences of the possible reduction of your activity.
  • Route of schooling, training and\or care:
    Items necessary for the evaluation are supplied by the teacher referent of the sector of schooling of kid.

  • Cards (incapacity, Priority or car park):
    It is useless to supply a passport photo during your deposit of file.

    - In case of agreement of disabled persons' card or priority, a photo will be asked you by Place - Handicap.
    - In case of agreement of an European card of car park, a photo will be asked you by the Departmental Direction of the Social Cohesion (DDCS ex--DDASS).
  • Clearing service of the Handicap (PCH):
    - Le Balance sheet of autonomy Filled, dated and branded with by your doctor,
    - The life plan (column B of the request form) allowing to describe your expectations,
    - Invoices and\or estimate bound to your needs (intervention of a help service at home, a purchase of materials, arrangement of the place of residence, the vehicle, the transport costs or other regular spending),
    - Audiogram recent and prescriptions for possible equipments.
  • Gratitude of the Quality of "Worker Handicapped person" RQTH and vocational guidance:
    - La Index card of career Freed by the MDPH,
    - If you are in job search: a CV or a copy of one or several diploma (s), copy of the registration to the employment agency, the documentary evidence of entrance to training if necessary.
    - If you are at present in employment: the index card of capacity of the filled professional medicine, dated and signed by the doctor of your company.
    - If you are at present in employment with difficulties of preservation for your post (because of the handicap): a form of accelerated request of RQTH can be joined to your file.
    This form must be filled by a company doctor only. He can be accompanied with a questionnaire to be filled by the concerned person:
  • Request form of accelerated RQTH To make fill by the professional medicine.
  • Questionnaire to be filled by the person in situation of handicap.
  • Manual To help to fill the request form of accelerated RQTH.


  • Orientation towards an establishment or a service:
    - Le Balance sheet of autonomy Filled, dated and branded with by your doctor,
    - The life plan (column B of the request form) allowing to describe your expectations,
    - If you are already taken care by an establishment or a service, an updated report (supplied by the team of the structure).

In case of loss, in case of flight or in case of deterioration of your European card of car park, the request of duplicate is to be sent to the Departmental Direction of the Social Cohesion:

Supply:

  • A mail of request with your address and phone number
  • A passport photo
  • The copy of your declaration of loss or flight with the commissionership
  • The copy of your notification of decision specifying the dates of validity of your card (in case of loss, the MDPH can free you a certificate of right).
